I've received an email with a link to provide feedback after my appointment. How do I fill this out?
Why did I receive this?
Many of the NHS clinics are now offering patients a chance to provide feedback on their appointment experience.
The clinics use the Swiftqueue system to provide the format for capturing this information.
When should I receive this?
- If the clinic uses this service, the below email (or similar) will be sent to the registered email address of the patient who attended their appointment usually 24 hours after their appointment.
- To provide feedback please click on the link received.
- After clicking the link, please login to your Swiftqueue account using your registered email address and password.
- Once you login, you will be brought to this screen
- Click on the Open Post Appointment Survey form and you will see a screenshot similar to below.
Please note: Each Trust/Clinic chooses the questions asked in order to capture their desired feedback.
- Once you have answered all the questions, click the Close button.
- Below screenshot will appear:
- Click the Finish button to complete
- Completed form will now show in your patient portal.
Having difficulty opening the form?
- Please check that you are logging into Swiftqueue with the correct email address which was used to book your appointment. If you login using a different email address, you will not be able to access / see this form.
